Yeah Right...

On an individual level, cannabis use confers an overall twofold increase in the relative risk for later schizophrenia. At the population level, elimination of cannabis use would reduce the incidence of schizophrenia by approximately 8%, assuming a causal relationship.

The strongest evidence that cannabis use is a contributory cause of schizophrenia comes from longitudinal studies of large representative samples of the population who have been followed over time to see if cannabis users are at higher risk of developing schizophrenia. The earliest such study was a 15-year prospective investigation of cannabis use and schizophrenia in 50,465 Swedish conscripts. The study found that those who had tried cannabis by age 18 were 2.4 times more likely to be diagnosed with schizophrenia than those who had not 16 and the risk of this diagnosis increased with the frequency of cannabis use.

Zammit et al 17 reported a 27-year follow-up of the Swedish cohort that also found a dose-response relationship between frequency of cannabis use at baseline and risk of schizophrenia during the follow-up. The relationship between cannabis use and schizophrenia persisted when the authors statistically controlled for the effects of other drug use and other potential confounding factors, including a history of psychiatric symptoms at baseline.

Zammit et al’s findings have been supported in a three-year longitudinal study of the relationship between self-reported cannabis use and psychosis in a community sample of 4,848 people in the Netherlands

These findings have been replicated in one German and two New Zealand cohort studies

When marijuana use triggers psychotic symptoms, the symptoms typically vanish once the marijuana has passed through the patient’s system. The condition, called cannabis-induced psychosis*(CIP), has a favorable prognosis. Total remission of symptoms can be expected in the short term. A number of recent studies, however, have demonstrated a link between CIP and the progression to schizophrenia in the long term. In one large study, for example, more than 500 patients were followed for at least three years after treatment for CIP. The researchers found that:

More than three-fourths of patients experienced psychotic episodes after the initial episode of cannabis-induced psychosis was resolved.

Almost half of the patients were subsequently diagnosed with schizophrenia. Often, the onset of schizophrenia occurred more than a year after treatment for CIP.

Compared to persons who had never been diagnosed with CIP, patients in this study developed schizophrenia at a significantly younger age.

Seventy miles northwest of New York City is a hospital that looks like a prison, its drab brick buildings wrapped in layers of fencing and barbed wire. This grim facility is called the Mid-Hudson Forensic Psychiatric Institute. It’s one of three places the state of New York sends the criminally mentally ill—defendants judged not guilty by reason of insanity.

Until recently, my wife Jackie*—Dr. Jacqueline Berenson—was a senior psychiatrist there. Many of Mid-Hudson’s 300 patients are killers and arsonists. At least one is a cannibal. Most have been diagnosed with psychotic disorders like schizophrenia that provoked them to violence against family members or strangers.

A couple of years ago, Jackie was telling me about a patient. In passing, she said something like,*Of course he’d been smoking pot his whole life.

Of course? I said.

Yes, they all smoke.

So marijuana causes schizophrenia?

I was surprised, to say the least.

The proportion of schizophrenia cases linked with problematic use of marijuana has increased over the past 25 years, according to a new study from Denmark.

"There is, unfortunately, evidence to suggest that cannabis is increasingly seen as a somewhat harmless substance. This is unfortunate, since we see links with schizophrenia, poorer cognitive function, substance use disorders, etc," Hjorthøj wrote.

Cannabis and schizophrenia/psychosis have a close relationship. We have evidence suggesting that cannabis use, primarily THC in cannabis, in genetically predisposed or at-risk populations, leads to earlier diagnosis of psychosis/schizophrenia. This tells us that THC in cannabis has a small causative effect on schizophrenia. THC in cannabis also makes schizophrenia and psychosis symptoms worse and causes more relapses and hospitalizations.

Researchers found strong evidence of an association between cannabis use disorder and schizophrenia among men and women, though the association was much stronger among young men. Using statistical models, the study authors estimated that as many as 30% of cases of schizophrenia among men aged 21-30 might have been prevented by averting cannabis use disorder.
