The preliminary report highlights that the incident at Brussels Airport last month involving Scandinavian flight SAS43M (SK2590) was even more critical than previously thought.
The aircraft had attempted to take off from a taxiway parallel to the assigned runway (7R) due to a combination of factors:
- An altered procedure being used, with the aircraft taking off from a runway intersection rather than using the entire runway length;
- The presence of a complex taxiway/runway intersection, designated as a hot spot;
- The aircraft not being equipped with Take-Off Surveillance 2 (TOS2), Airbus Runway Overrun Prevention System (ROPS), nor RAAS (Runway Awareness and Advisory System);
- Absence of natural lighting, the incident taking place after civil twilight and before the moon rose;
- Runway and taxiway surfaces being wet, making it difficult to read runway markings in darkness;
- Illumination of a crucial intersection sign being inoperable at the moment of the incident;
- The red stop bar at the taxiway/runway intersection extinguishing prior to the crew's arrival at this point, robbing them of a visual reference point;
- Tower and ground frequencies being operated by a single traffic controller at the moment of the incident, whose attention was divided between 7 different aircraft at the moment of the incident.
The aircraft entered a parallel taxiway instead of the assigned runway and attempted a take-off, which was only rejected at a speed of 127 kts, just shy of the V2 reference speed of 132 kts, after which the aircraft came to a full stop in just 14 seconds. The crew initiated a right turn while still at speed when they realised that the remaining length of taxiway ahead of them was closed off with temporary fences. The aircraft came to a halt on asphalt pavement adjacent to a crossing taxiway, with its front landing gear just meters away from runway guard lights.
At the moment the crew abandoned their take-off, they only had approximately 520 meters/1705 ft of taxiway available, after which the aircraft would have collided with the temporary fences.
The final report is expected to be published in about a year's time.
